ScreenBill counts only time spent inside apps. Lock screen, home screen and system UI time are left out, so the total usually reads slightly lower. In exchange, the line items always add up exactly to the total.
No. ScreenBill does not block anything. It gives you slabs, a bill and a nudge. What you do with the bill is up to you.
